I have one in which main income comes.
A couple others for dealing with transactions with friends and family.
A frozen account due to a chain dispute.
And some small wallet accounts like easypaisa and stuff.

Yes, when registering with FBR you are required to declare all active bank accounts under your name, not just the one where your main income comes.
Accounts used only for personal/family transactions also fall under this requirement.
Frozen accounts are normally still reported, since they legally exist in your name.
Mobile wallets (like Easypaisa/JazzCash) also count as accounts if linked with your CNIC.
Non-disclosure can create issues during audit or cross-verification by FBR.
